It does not mandate review timing, assign responsibility, create legal conclusions, or prescribe institutional action.

These checkpoints are reference prompts for post-incident clarification only.
They may be used when reviewers need to separate immediate facts, revised assumptions, and later institutional changes.
Checkpoints
1. Immediate review
What decisions were made within the first 72 hours?
Was a human refusal pathway identifiable before irreversible impact?
Were relevant records preserved without retrospective modification?
2. 30-day review
Which assumptions were revised or withdrawn?
Did any temporary explanation become treated as precedent?
Were missing records, unverifiable refusal pathways, or AI-output-as-final-rationale issues identified?
3. 90-day review
Which institutional processes changed, if any?
Did responsibility attribution remain human and traceable?
Were conditions for effective refusal before future irreversible impact restored or clarified?
