Based on the available records, effective human refusal before irreversible impact has not been demonstrated.
This does not by itself determine legal liability, technical fault, or regulatory violation. It means that, under the LUMINA-30 boundary lens, the case should not be described as procedurally valid on the basis of human oversight alone.
If the process is to be resumed or repeated, the refusal pathway should be restored, documented, and made operationally effective before restart.
