This document checks whether AI-generated output became the sole or primary rationale for a final action, decision, or approval.
Core Concern
Human approval is not sufficient if the human decision merely ratifies AI-generated reasoning without independent refusal capacity.
Check Items
1. Sole Rationale
Was the final decision based solely on AI-generated output?
2. Primary Rationale
Was AI-generated output the primary rationale even if a human formally approved it?
3. Independent Human Reasoning
Was there an independent human rationale recorded before final action?
4. Refusal Option
Could the human decision-maker reject the AI-generated rationale without procedural penalty or practical impossibility?
5. Final Approval Gate
Was there a final approval gate that required independent human responsibility before action?
Boundary Assessment
If AI-generated output became the sole or primary rationale for final action, effective human refusal is not demonstrated unless independent human refusal remained operationally available.
