Minimum Decision Rule
If the review cannot identify who is responsible for friction design, operation, evidence preservation, verification, and correction before irreversibility, the boundary condition is not adequately preserved.
If evidence that effective refusal remained practical cannot be shown, do not infer effectiveness from silence, absence of complaint, or nominal procedure.
