A YES, NO, UNKNOWN, or NOT RECORDED answer is not an automatic pass/fail result. If the answer is NO or UNKNOWN, record what must be clarified or resolved before proceeding.

Suggested wording

This is not a request to block the project. It is a way to make the decision record reviewable later.

If challenged

  • If someone says “we already have human oversight,” ask whether humans can effectively refuse, stop, verify, or redirect before irreversibility.
  • If someone says “this will block innovation,” clarify that the question records reversibility before dependency becomes hard to unwind.
  • If someone says “legal or security review already covers this,” clarify that this question does not replace those reviews; it adds a record of effective human refusal before irreversible escalation.
  • If the answer is UNKNOWN, the next step is not blame. The next step is to identify what must be clarified before proceeding.
